DIAGNOSTIC DILEMMA
Pediatriconcall
A 10 years old boy born of third degree consanguineous marriage presented with gradual onset weakness of lower limbs since 2 years. He had a generalized tonic convulsion at 5 years of age and then repeat convulsions every 15 to 30 days till 7 years of age. He is convulsion free since last 3 years. Similar complaints of weakness and convulsion is noted in elder sister now 12 years of age and had onset of disease at 5 years of age. What is the diagnosis? Read more…

Colour Doppler did not show any collaterals initially but repeat colour doppler by the same sonologist after 5 days showed presence of portal cavernoma. Query:An 8 month old boy presented with multiple episodes of generalized tonic clonic convulsions On examination he had right sided hemiplegia without any cranial nerve involvement. What is the cause of the hemiplegia and where is the CNS lesion?
Answer:
Right temporal infarct and workup for thromboembolic phenomenon revealed Protein S deficiency.
