This month’s teaching File:A 10 month old boy presented with fever and vomiting since 1 month. He had hard stools and increased thirst. At 7 months of age, he received intramuscular vitamin D (6,00,000 units) and calcium supplements. Examination findings were normal. What is the diagnosis? Query:A 4 year old presented with splenomegaly, hematemesis and malena. Colour Doppler did not show any collaterals initially but repeat colour doppler by the same sonologist after 5 days showed presence of portal cavernoma. Why did the earlier colour doppler not pick up portal cavernoma?
Answer: The hematemesis relieved the portal pressures and so the cavernoma was not picked up. With refilling of the portal system subsequently, the cavernoma was picked up. Read more... phenomenon revealed Protein S deficiency.

Vein of Galen Aneurysm (VOGA) is a rare congenital malformation of brain. Most cases present in neonatal life with congestive cardiac failure.
