11 year old girl came with complain of pyrexia of unknown origin, pain in abdome,vomiting.lab test for malarial parasite,,montux,csf,chest-xray,usg abdomen all reports were negative,her initial diagnosis was enteric fever as widal was mildly positive{1:20,1:60} & leucocytic count was 3000 thats why monocef was given for 1wk.but no response,treatment for malaria was given suspecting falciperum malaria as anemia was continously increasing.again no response,after repeating all the test usg abdomen showed mild asits,splenohepatomegaly lymphadinopathy at this level her cns involved & she was neither opening her eyes nor responding to talk even not for micturation reflex,{responding only for pain},her ct scan shows mild widening of cortical sulci,2echo shows mild left ventricular hypertrofy,{2pind pcv is given}now her ANA was done which is positive{4.38}now patient is well no fever no vomiting,responding for talk, what to do further???? Dear sir 16y old female present with painful petechial eythematous lesion in both lower legs seen by dermatologist, diagnosed as vasculitis.The patient gave history of GN 6y ago.Now she is on captopril+propranol+prednisolone for the last 3y. Prior to that she was given cyclophosphamide for 4months. On exam. Obese healthy, vital sign are normal,GUE is nomal apart from trace albumine,renal function are normal. My questions are; 1.Any relation between vasculitis and activity of the disease . 2.Can we taper the treatment or does she need treatment now. . 3.Is renal biopsy indicated. Iam waiting reply thanks
