4 days young normally deliverd-cryed immediately after 2nd in birth order neonate to ANC Reg. mother having persisitant 102 degree celcius fever since day 2 of life. Maternal history of intermediate white per vaginal dischare during ANC for which due treatment taken & no other problems where faced.on phsical finding= no refusal of feeed-taking adequate frequent breast feeds,normal voidings,no convulsion, no any abnormal features or rash. on investigations Hb-17.9,total leuko-10800,poly-67,lymp-30,eo-7,mono-2,Platlets-210000,CRP 2.6,sgpt-17.8,CREAT-1.6. on treatment with-chloroquine,taxim-sulbactum,paracetamole. insted of which fever not coming down. kindly suggest your valuable guidances for further needful diagnostic & managment purpose as soon as possible     Check out the answer

4.5yrs young girl of 16 kgs came from shcool & develop 103 Temp with chills & typical febrile convulsion. no any abnormal physical analysis.all blood investiation suggest tc 9500;p73;l23;e 01;m 01;plat 210000 crp 6.2; MP-neg. CXR-prominant broncho-vasculor markings,widal-normal. pt. hospitalized & emperically started with ceftriaxone & artemether! & fewere came down to 100 with in 2 days but again on day 4 she got 103 tem. the repeat investigations were done for MP was normal with Hb 12.1,TC 4700;P42 L 56 E 01 M 01,ESR 15, CRP 9.9, & Urine- Normal.clinically pt occasionaly complians of pain in abdomin orelse inbetween fever pt is plyful but has loss of appetite & diet;does not appear very sick. kindly suggest possible diagnosis & managment/investigation (can it be Leptospira?) ?     Check out the answer

Have anyone tried Paracetamol (Anamol) suppositories launched by Elder Pharmaceuticals on their paediatric patients. I have tried it on 3 patients and it gave an excellent results in Fever associated with teething and immunisation. Female of 1 year with fever of 10 days continous high grade. No other complaints. No cough/vomiting/diarrhoea temp 101 to 104 deg f g/e nad s/e nad inv 1st day tc 25000/dc:p80l20 mp neg/crp 38.5 10th day tc 32000/dc p90l10 mp neg crp 39.5 in between cbc/mp/crp repeated twice was almost same patient on ceftrioxone + amikacin for initial 3d now on megapime + gatiflox 7d fever still present no other complaints in betweeen fever pt is fine /normal activity/ playful/ eats well. Dear sir I want to ask you few questions in the field of pediatric. We were isolated from world for 15 y due to previous embargo and now by U.S occupation. I have lost2 of my patients 2months ago with 1 month apart between them.Both patients are below 6 month presented suddenly by high fever ,grunting apathy,poor apetite. On exam. nothing apart from Fever 39.5c Tachycardia Drowsy Hepatomegaly 2f below costal margine. Over few hours sudden deterioration ,convulsion then passed. Investigation; normoglycemic .and due to our miserable condition nothing is available . Treatment; i.v fluid ,antibiotic oxygen What are the most likely diagnosis? A 14 yrs old young female presented to us with high grade fever since last 20 days. She had calf pain for 3-4 days during illness. On 4th day of admission she developed generalised erythematous maculopapular rash with itching. No other positive history. No H/O bone pain, any tenderness, altered sensorium. No LNpathy systemic exam- Normal. Her CBC showing polymorphonuclear leucocytosis with total counts upto 22000/mm3. CRP 42. LFT, KFT, Na/K, HIV/HbsAg, RBS- normal Sr uric acid, CPK- Normal Montoux Widal : Negative ANA RA factor Urine R- Normal Bone marrow aspiration - Normal CxR & USG abdomen- Normal Despite giving cefuroxime & amikacin & paracetamol, she is getting continuous high grade fever. In view of drug fever we stopped all drugs for 72 hrs but fever has not come down. What might be the possibitity, next investigation?     Check out the answer
