Can one set of twins be HIV positive and the negative?

Yes, it is possible if the transmission of HIV has taken at the time of delivery or while breast feeding. At the time of labour there is 13-20% chance of infection and another 20-30% risk with breast feeding. Placental transfer of HIV can occur and can affect one twin if the the twins are dizygotic whereas if monochorionic monoamniotic and intrauterine transmission through placenta can occur in both twins.
