Question

A 4 months male baby has taken 4 doses of hepatitis-b vaccine.one after birth,next at 6 wks ,10 wks,&15 wks.what will happen as he taken an extra dose of hepatitis-b vaccine? What will be the best management in gerd(pediatric age)"? Should mosapride be used in a child of 2yrs 6 months? Should flu-vaccine be given to all children?

Answer

Regarding the first question of 4 doses of HepB vaccine given to a baby,it will not cause any adverse effect.In fact the recommendation by WHO for countries where perinatal transmission is high is to start first dose at birth specially for India and to follow it up with 6 and
14 weeks or 6,10 and 14 weeks schedule.The last schedule is an
accepted one specially in country program situation where it may not
be possible to remember to give 2 doses following the 0 dose by a health workeralong with DPT and OPV .There may not be immunisation card always avilable with the parents.Hence 4 doses schedule can be followed.
GERD in infants can be managed by thickened feeds, propped up position while feeding, prokinetic agents under the guidance of a gastroenterologist and if severly sympotmatic by surgery.
Regarding Flu vaccine, it should not be given routinely to all infants since we are not sure about the serotype prevalent in our country and the efficacy of the vaccine is also about 60% only.
