Dr. Dr Vinod Asks
there is case of four months old ,it is seventh child of non consigious marriage. Baby presented with bleeding manifestation in the form of echymosis ,costochondral bleeding, pallor, not achieved any milestones till.ON blood examination HB was 5 Percent , Platelet count was normal, and pt and aptt both were prolonged.
On detail history it was non consigous marriage,only one leaving child was there

Dr. mm desai says,
Late Hemorrhagic Disease of New born is a possibility
Rx with inj vit k 2 mg and repeat Pt, PTT after 24 hrs if corrected it is likely to be Late HDN.
if no correction occur then r, o common pathway coagulation factor deficiency like Factor X, V fibrinogen and prothrombin need to be ruled out.
Do thrombin time if prolonged likely to be Fibrinogen deficiency in which cas BT is also prolonged
