CHANGING PATTERN OF URINE MICROBIOLOGY AND ANTIMICROBIAL SENSITIVITY IN A TERTIARY CHILDREN HOSPITAL

Introduction
Urinary tract infection is a common infection in children. Early, appropriate and adequate treatment prevents parenchymal damage and its consequences of scarring, hypertension and renal failure. Knowledge of the prevalence of various micro-organisms and its sensitivity to antibiotics would ensure early and appropriate antibiotic therapy. However in recent times, it has been observed that the prevalence of organism has undergone a recognizable change and its sensitivity to various antibiotics have also remarkably changed. With the emergence of ESBL organism, it has become a more difficult task. This study is conducted to study epidemiology of micro-organisms in urine in last 5 years and antibiogram.

Subjects & methods
This study was conducted at the departments, pediatric medicine, microbiology and nephrology at Kanchi Kamakoti Childs Trust Hospital, Chennai-34. Urine culture reports (organism isolated and antibiotic sensitivity pattern) of children were retrospectively analyzed from the period 2000-2005.

Results
E.coli is the commonest organism isolated. There has been a steady increase in the ESBL rates over the last 2 years with E.coli ESBL contributing to 30% to be followed by Klebsiella ESBL contributing to 7%. The sensitivity of the Non-ESBL organisms to Quinolones, Aminoglycosides and 3rd generation cephalosporins is 100% while ESBL organisms are sensitive only to Cefaperazone-Sulbactum and Nitrofurantoin.

Conclusion
ESBL organisms are on the rise. The reason may be due to over use of 3rd generation cephalosporins. Hence judicious use of antibiotics is important. Correct choice of antibiotic is important when we are dealing with ESBL organism growing in the urine of a sick child with suspected upper UTI.
