Diagnostic Dilemma - Pediatric Oncall

A 3 years 8 months old child presented with recurrent cough and cold. His grandmother was on antituberculous therapy {ATT} for past 6 months. For the same, the child had been started on ATT for past 6 months but symptoms persisted. His Mantoux test was negative. Chest X-Ray at start of ATT showed collapse consolidation of left lung with shift of mediastinum to same side. Current X-Ray also showed similar findings but a cavity within. A sputum culture did not show any acid fast bacilli on smear. Child was given total 1 year of ATT and then it was stopped but Chest X-Ray was same.

Answer :- some other cause of collapse should be find out...CT scan of chest should be done

Answer :- I think this is a residual disease {atelectasis, fibrosis}. A chest HRCT should be obtained.
