LOWER RESPIRATORY TRACT INFCTION - HOW TO INVESTIGATE?
Question:
What is the possible diagnosis in this child?
Answer:
This child has presented with complaints of cough and cold to begin with which has now led to tachypnea. There is no fever. Child is not toxic. Hence one should suspect a viral infection rather than a bacterial infection here. Since the chest appears to be hyperinflated with obliteration of cardiac dullness and hyperresonant note over the lungs, suspicion of bronchiolitis is very strong especially considering the age group and symptoms in a well-nourished child.
Investigations:

Chest X-ray: shows hyperinflation with narrow heart.

Hemogram: Hemoglobin- 12gm%, White cell count - 12,450/cu mm (40% polymorphs, 60% lymphocytes), Platelet count - 2,50,000/cu mm, ESR - 12 mm at end of 1 hour

C-Reactive Protein - negative

Blood culture - no growth
