DIAGNOSTIC DILEMMA

A 15 year old presented with generalized swelling, which started with periorbital puffiness and hematuria for 5 days. On examination he had edema with hypertension (BP 170/100). Fundus examination was normal. His serum urea and creatinine were 85 mg% and 1.1mg/dl respectively. ASLO was raised and serum C3 level was reduced. On urinalysis,RBC count was 250/ml, proteins 4 plus and leucocytes 100/ml.

Shall we categorise him to the hypertensive emergency or hypertensive urgency? If it is to be categorized as hypepertensive emergency, shall we treat this with calcium channel blockers and frusemide or aggressively in PICU with intravenous antihypertensive drugs i.e. Sodium nitroprusside or labetalol under constant monitoring?
