DIAGNOSTIC DILEMMA
Pediatric Oncall
A 7 years old girl presented with fever for 1 week and anorexia and weight loss for 3 months. She was on Antituberculous Therapy {ATT} since 3 months in view of Chest X-Ray suggestive of Miliary TB with positive Mantoux test. However drugs had been stopped for past 10 days due to epigastric pain and tingling numbness. On examination, she was malnourished, had hepatomegaly with bilateral crepitations. Investigations showed multiple cystic lesions in bilateral lung fields with pneumonia on right side and multiple small abscesses seen in both lobes of liver. HIV ELISA was negative.

What is the diagnosis?

A 3½ years old girl is in contact with an adult suffering from XDR-TB. Her Mantoux test is also positive. She is otherwise asymptomatic. Her Chest X-Ray is normal.

How should this child be treated?

Last month's teaching file: A 4 month old boy, presented failure to thrive. Mother had fever at 7 months of gestation. The child on Day 15 of life developed rash over body with fever which cleared off in 4 days. At 2 months of age, he had hematemesis with malena and was given whole blood transfusion and fresh frozen plasma. On examination, he had pallor, hepatomegaly. Investigations showed elevated liver enzymes and positive CMV IgG with CMV viral load = 3000 copies/ml
Does the CMV infection require treatment?

Answer: Rreatment is recommended only when there is symptomatic CMV disease. Active CMV organ dysfunction is not found in the child...
