A 4 year old presents with recurrent vesicular rash on the face for past two years. Skin biopsy- non conclusive. The child responds to oral or iv antibiotic therapy each time the rash appears. Is it recurrent impetigo due to some rare immunodeficiency?

A pregnant woman at 8 months gestation underwent a regular antenatal ultrasound where it was found that the fetus had mild ventriculomegaly. She then underwent a malformation scan and it was found that the child had Dandy Walker variant with subependymal calcifications in the brain with normal abdominal scan. The mother had fever at 3 months of gestation. A TORCH titre was done in the mother and CMV IgG was positive with absent CMV IgM. Karyotype was not advised as there were no other abnormal features. What should be further management?

Last month's teaching file: A 2½ year old boy presented with fever, maculopapular rash and a generalized tonic clonic convulsion. On examination, he has tachycardia with hypotension and prolonged capillary refill time with hepatomegaly. There are no meningeal signs. What is the diagnosis ?


Answer: Fever, rash and convulsions.
