DIAGNOSTIC DILEMMA

A 5½ years old boy presented with recurrent fever and jaundice for 1½ months, with right ankle pain for 7 days. He had Widal positive (0 titre= 1:160) and was treated with cefoperazone for 10 days to which fever responded. However again for past 7 days, he had fever, jaundice and ankle pain. He was started on piperacillin-tazobactum and clindamycin to which fever subsided in next 48 hours and LFT also showed improvement. CRP also decreased to 68 mg/dl.

A 12 years old child, a diagnosed case of RHD with MR with MS with mild PH presented with increasing breathlessness since 8 days. On examination, he had irregularly irregular heart rate of around 160/min, respiratory rate of 44/min, blood pressure of 92/60 mm of mercury, cardiomegaly and hepatomegaly. Pre treatment ECG Lead II (Figure 1) is depicted below.

Last month's teaching file: An 18 months old child presented with pulmonary TB and HIV disease. He had failure to thrive {height = 71 cm, weight = 7 kg} with delayed development and hepatosplenomegaly. He was started on TMP-SMX prophylaxis but developed urticaria and thus was stopped. His CD4 count was 30 percent. What is the diagnosis?
