DIAGNOSTIC DILEMMA
Pediatric Oncall

A 4 months old boy presented with fever and cough for 7 days. His chest X-Ray had shown right upper and mid zone consolidation for which he had received antibiotics for 7 days but there was no response. He underwent a HRCT chest that showed lobulated mass involving the entire mediastinum with extension to the chest wall on the right side and displacing the lungs suggestive of either a germ cell tumor or a thymoma. A CT guided biopsy was done that showed no identifiable tumor cells and only inflammatory pseudotumour. He was started on antituberculous therapy in view of persistent fever to which there was no response.

What should be done now for this child?

A 3 year old male child born of non consanguineous marriage presented with bilateral cataracts since 10 months. Birth history and milestones were normal. There was no jaundice, failure to thrive or dysmorphic features. On examination, apart from bilateral white reflex, other systems were normal.

A 10 month old girl child came in our out-patient department with complains of dark circles {blackening} around both eyes and occasional fever. On examination, the child was afebrile, markedly pale and very irritable. She also had periorbital hyperpigmentation and ecchymosis and mild proptosis bilaterally {Figure 1}. There was no history of trauma or abuse. On abdominal examination, liver was palpable 2cm below the right subcostal margin. What is the eye lesion called?
