DIAGNOSTIC DILEMMA
A 5 years old boy was detected to have endobronchial TB at age of 2 years and was on antituberculous therapy (ATT) for past 3 years. Currently, he had pneumonia with otitis media and molluscum contagiosum. HIV ELISA was positive. CD4 count was 509 (23%) cells/cu mm. CT chest showed endobronchial spread of disease. Child was started on Category 2 of ATT and bronchoalveolar lavage (BAL) was sent for TB Bactec culture. Patient was subsequently lost to follow up for next 6 months. He again presented with molluscum and chest X-Ray showed same picture.

How should this child be managed?

A one year old developmentally normal girl was brought with fever and left focal convulsions. Her blood counts, electrolytes, calcium, blood sugar, urea, creatinine, cerebrospinal fluid analysis were all normal. Detailed ophthalmological and fundus examination was normal. She had multiple episodes of convulsions in the hospital and was controlled with carbamazepine and phenytoin. There hemangioma seen over right upper eye lid, forehead and lateral side of skin over the nose. She had no motor deficits. Spot Diagnosis
