DIAGNOSTIC DILEMMA
Pediatric Oncall

A 1½ years old boy had chronic hepatitis. His Hepatitis A,B,E,C markers, ANA, anti smooth muscle antibody, anti LKM were negative. Liver biopsy was non-specific. He continued to have elevated liver enzymes and at 4½ years of age, he presented with difficulty to get up from sitting position with calf hypertrophy and positive Gower?s sign. His CPK was 15,560 IU/L and EMG showed primary myopathy.

Is it Duchene muscular dystrophy or is it glycogen storage disease?

A 4 month old boy, presented failure to thrive. Mother had fever at 7 months of gestation. The child on Day 15 of life developed rash over body with fever which cleared off in 4 days. At 2 months of age, he had hematemesis with malena and was given whole blood transfusion and fresh frozen plasma. On examination, he had pallor, hepatomegaly. Investigations showed elevated liver enzymes and positive CMV IgG with CMV viral load = 3000 copies/ml

Does the CMV infection require treatment?

Last month's teaching file: A 2½ years old child born of third degree consanguineous marriage presented with gradual abdominal distension and jaundice since 1½ months. She was seen by a physician 1½ months back and treated with steroids and also received a blood transfusion. There is high coloured urine. Milestones are normal and birth was uneventful. She is immunized till date but has not received Hepatitis B vaccine. On examination, she has steroid facies, is normal nourished, has jaundice, hepatosplenomegaly and other systems are normal.
What is the diagnosis?

Answer: Alagille syndrome
