I have a 4 year old admitted at present in pediatrics ICU ventilator. He is known to suffer from bronchial asthma, also known as brittle asthma. He came in with a very bad attack of asthma with respiratory acidosis and was ventilated. Now he is still ventilated and is on iv hydrocortisone ,Iv salbutamol infusion 4 mcg/kg/min , iv terbutaline prn basis (when has desaturation secondary to bronchospasm),nebuliser salbutamol two hourly to hourly , MDI salmeterol 100 mcg bd,MDI inflammide 800 ,cg bd and he still goes for bronchospasm on and off usually secondary to secretion. The secretion are not much. What else can i do for him? He does not seem to be heading to the way of improvement. Will MGSO4 helpful to reduce the bronchospam in him?

SIR, my son ,7 years old, is asthmatic. He is on inhaled fluticason {100} by discus, twice per day. On missing the dose, it leads to an attack.he was classified as moderate persistent asthma. I want to know for how long will he need to use this drug? He washes his mouth after each inhalation. I want to know what is the effect of this drug on his teeth? I noticed one permanent tooth is not fixed and it moves in its place. Do I need to give him calcium? Thank you.

Sir, My doubt is about the grading of asthma. Asthma is a disease, where the symptoms such as cough and wheezing are more at night compared to the day time. But when we look at the grading, it appears that the night symptoms are comparatively less frequent than the day symptoms. For example, in moderate persistent asthma, criteria for symptoms - are daily symptoms and for nocturnal symptoms it is more than once a week. If a person is having daily asthmatic symptoms, won’t he be having daily nocturnal symptoms as well? To meet the criteria of mild persistent asthma, you need to have symptoms more than once a week, but need to have the nocturnal symptoms more than twice a month only. According to the standard teaching, if someone is having asthmatic symptoms more than once a week, he will definitely have night symptoms at the same or increased frequency.
