As a resident in a tertiary hospital how should I manage various congenital heart diseases referred to us from other hospitals in the emergency room before I refer them to pediatric cardiology for intervention? I wish to know to which lesions, I should administer oxygen and Prostaglandins as emergency?

Dear Sir, My daughter born 2 months back is diagnosed as a case of down syndrome with AVSD- PS(TOFF).Her Echo report asd+av canal defect vsd type+severe PS&IS+hypoplastic pulmonary annulus. Normal Biventricular function..At present she is having cynosis in feet & hand on crying. Her oxygen saturation 75% TO80%.At birth her weight 3 kg now she is 4.2 kg. What is prognosis & progress of cardiac disease?

I have a patient 7 months old ,7.750kg,66cm with vsd, subaortic-small but with severe pht, presented with tachypnea, hypoxia, cxr reveal diffuse small round opacity, fever, cefotamim, ampiclox, garamycin. 1 week no response. Vancomycin started, 4 days no response, hypotension metabolic alkalosis, hypokalemia, treatment accordingly refadin stared last day. Last day investigation reveal hb 6g/l, Retic 1.5% B. Urea 70g/l ,Cr 9, Na 115, Ca 6, K 3.5, tp 4.6, alb 4.2, sgpt 40, sgot 70, fever subsided, hypoxia improved. We put a dx of resistant Staph. aureus. Any opinion?

My sister is 28 yr old , now 27 weeks pregnant with fetus having complex cardiac anomalies (endocardial cushion defect , VSD ,PS,double outlet right ventricle).she had past history of abortion twice ,first was medically induced , second was spontaneous both in later half of 2003. No other significant family or past history. Kindly guide us regarding management of this at the earliest. yours sincerely, Manish.
