Question
In childhood asthma cases we generally put patients on preventers in form of inhalers. My query is how long these children should be continued on preventer inhalation therapy? This is a question generally asked by parents.
Answer
Once you start a child on inhaled steroids for prevention of asthma, there is either a step down or step up dose depending on the symptoms of the child. What we do is, start the child on a high dose steroid and follow up the child regularly. If the child has been asymtomatic, we step down the dose every 3 monthly till a plateau dose can be maintained. If the child has been asymptomatic for 1-2 years on low dose, we omit the inhaler and watch for recurrence of symptoms as several children outgrow the disease as they grow older. However, this step up and step down is an individual preference.
