14 y female present with pallor and tachycardia her present and previous ECG is;follow this link http://img224.imageshack.us/img224/733/picturedi8.jpg What is the diagnosis?.? How do you manage her ??
Answer
ECG definitely shows tachycardia for the age with narrow QRS complexes s/o SVT.
Additionally there is positive QRS in aVL.Hence an echo would be necessary to r/o silent ebstein or corrected TGA.
Some leads are show WPW phenomenon , hence a complete ECG with long lead II essential to r/o wPW syndrome.
Best treatment would be tab. amiodarone or propranolol under supervision. Considering her age one may have to r/o brugada syndrome i.e RVOT cardiomyopathy
Commonly seen in girls and part of ? autoimmumity .
