Question
As a resident in a tertiary hospital how should I manage various congenital heart diseases referred to us from other hospitals in the emergency room before I refer them to pediatric cardiology for intervention? I wish to know to which lesions, I should administer oxygen and Prostaglandins as emergency?
Answer
Oxygen is required in all congenital heart disease even if cyanotic if they are in failure or in cyanotic spell. Prostaglandins are required in all patients with duct dependent circulation.

Duct-dependent for systemic blood flow (e.g. Hypoplastic Left Heart Syndrome, critical aortic stenosis, interrupted aortic arch).


Duct-dependent cyanotic lesions (e.g. pulmonary atresia, transposition of the great arteries)

Over oxygenation should be avoided.
