Question

Dear Sir, My daughter borned 2 months back is diagnosed as a case of down syndrome with AVSD- PS(TOFF).Her Echo report asd+av canal defect vsd type+severePS&IS+hypoplastic pulmonary annulus.Normal Biventricular function..At present she is having cynosis in feet& hand on crying.Her oxygen saturation 75% TO80%.At birth her weight 3 kg now she is 4.2 kg.What is prognosis & progress of cardiac disease

Answer

Cardiac - i would like to start 1mg/kg/dose twice a day on ciplar i.e 1/2 tablet of 10 mg twice a day.Monitor the Pulse Oximetry at 15 days interval. if pesistently > 75-80% do a complete surgical repair at a good centre ( Kochi, bangalore, hyderabad, asian heart) by 6 months time.Prognosis for heart is good as biventricular functions are normal.

The child presently should be immunised for hepatitis b . Also T3,T4 , tSH for hypothyroidism, USG abd for kidney anomalies and karyotype need to be evaluated.Child need infective endocarditis prophylaxis ( antibiotics) if she has to to undergo some
