My son, is 4 months old. During birth (scissor Ian baby)his weight was 2.2 kg,all other symptoms found normal. At his age of 1 month and 11 days,he had undergone Hernia operation. Latter, it was found that he has murmur (by stethoscope). Now he is 4 months old.His wt.is 5.3 Kg. No abnormal symtom is found from outside. The latest Echocardiography report is enclosed below which shows he has MPA stenosis with proximal RPA stenosis and supravalvar aortic stenosis We are very much worried for the same.Can you suggest the possible rectifying measures to be taken to eliminate the defect so that he can live normal life in future. REPORT OF ECHOCARDIOGRAPHY WITH COLOUR DOPPLER STUDY Date :21/01/02 Patient : Sayan Banerjee Age : 4 months Impression: 1.Situs :Solitus 2.Veins,atria,inter-atrial septum :IVC to RV,SVC to RA,Pulmonary venis to LA.Intact IAS. 3.Atrio-ventricular valves,Ventricles,Ventricular septum:No TR,MR.Normal RV: not dialated or hypertrophied.Good RV,LV .Intact IVS. 4.Semilunar valves,Great arteries:Normal aortic valve and pulmonary valve.Distal MPA stenosis with proximal RPA stenosis. Peak gradient = 35 mmHg. Flow related LPA gradient =30 mmHg. Probable supra aortic valvar stenosis gradient = 30mmHg. 5.Arch,Others :normal arch. Conclusion: 1.MPA stenosis with proximal RPA stenosis. 2.Supravalvar aortic stenosis(mild).   

Answer

Since your child has multiple heart lesions, a cardiac catherization would be required to know the hemodynamic condition of the heart. Depending on the cardia cath report further treatment can be advised. Pulmonary stenosis can be treated with ballon dilatation whereas treatment of aortic stenosis will depend on the cardiac cath report. We would also like to know if your child has some dysmorphic features as Pulmonay stenosis and aortic stenosis together are seen in William's syndrome.
