What is the treatment and its duration for a case of Tropical Splenomegaly Syndrome in a Malaria endemic area?
Answer
Treatment of tropical splenomegaly consists of antimalarials such as chloroquine (10 mg/kg base PO followed by 5 mg/kg 6, 24, and 48 hours later and then weekly for as long as it takes for response to occur), proguanil or pyrimethamine. The treatment have to be given for a prolonged time till serum IgM levels revert to normal, spleenic enlargement disappears, improvement in general well being and improvement in anemia.
