Question
1)If mantoux test has been done in patient recently, can P.C.R/ELISA be done in this Patient? 2) what are the indications of PAM in OP Poisoning? Are there any therapeutic guidelines in this regard?
Answer
There is no contraindication to PCR or ELISA test after mantoux test.

PAM is used as an antidote to reverse muscle paralysis as it reactivates the phosphorylated AChE by binding to the OPC. It is ineffective once the OPC has bound AChE irreversibly and hence needs to be administered within 48 hours of exposure. Dose recommended is 20-40 mg/kg IV in 100 ml NS/D5 over 15-30 min to be repeated in 1-2 hours if muscle weakness is not relieved. PAM can precipitate myasthenia gravis and muscle rigidity in healthy volunteers.
