Respected madam, cefoperazone+sulbactum by IPCA (lactagard) has 2:1 proportion and old brands were with 1:1 ratio(zostum). which is a rational combination?
Answer
Sulbactam combines with some clinically relevant beta lactamases in an irreversible manner. If sufficient inhibitor is present at the site of infection, the beta-lactamase enzymes should be neutralized and thus the drug used in combination with the inhibitor should have an opportunity to inhibit bacterial growth. 2 parts cefoperazone combined with 1 part sulbactam ratio resembles the expected levels in blood shortly after administration. The half-life of cefoperazone is much longer than that of sulbactam and thus the ratios will change with time.
