Question
What is the dose of steroids in septic shock? Is it 10mg/kg or more than that? Do we also add fludrocortisone along with hydrocortisone for better effects in managing the shock??
Answer
Administration of steroids (e.g. methylprednisolone, hydrocortisone, dexamethasone) has theoretical benefits in the setting of severe sepsis by inhibiting the massive inflammatory cascade that is unleashed. Thus more anti-inflammatory action is required as compared to mineralocorticoid action. Choice is between hydrocortisone 2mg/kg/dose or dexamethasone 0.1mg/kg/dose.
