Can cefepime be used in enteric fever? a 5 yrs female child is having high fever(106f)since 7 days.crp --24units(normal-6 units)twbc--6100,esr-10,urine --pus cells:av.5-6/hpf.widal --1 :120(o)@1:240(h).cxr--nad.soft splenomegaly,no hepatomegaly.pt. non -toxic.what is the probable diagnosis?and management.

Since WIdal is borderline and patient is non-toxic, one needs to be sure about the diagnosis of enteric fever. A clot culture would be useful. Also rule out malaria.
For enteric fever, third generation cephalosporins such as ceftriaxone would be a better choice of drug.
