12 yr girl, HIV +,family H/o TB +, had symptoms suggestive, had pulm. Infiltrates and abdomen . LNpathy, FNAc of which showed AFB +. CD4 was 202, sr.albumin was 1.8. child was also started on efavirenz based ART. 3 months after starting HRZE child has not shown any improvement, has developed ascites and pedal edema. Present sr.alb is 1.6. ascitic fluid protein is 136 mg and there are 160 cells 80% lymphocytes. What should be further management of this child?
Answer
Send the ascitic fluid for Tb culture sensitivity and cytospin test. Also what is the situation of her pulmonary symptoms? Make sure EFV is increased by 15% of recommended dose when the child is on treatment with rifampicin. Also monitor liver function tests. One may consider making Anti Tb treatment into 6 drugs HRZE + Streptomycin + Ciprofloxacin till culture reports come.
