A 2 years 7 months child is still having open anterior fontanelle. It is pulsatile, not bulging. head circumference is normal. Intellect is normal. What investigations should we do? What are the d'd? 2. a child has taken 3 doses of antirabies 10 months ago. Again he has taken 5 doses of rabipur 1 month back. will it do any harm? Is there any treatment for mesenteric lymphadenitis?
Answer
Since the milestones are normal, one may consider the following D/D:

Pyknodysostosis, Russell-Silver Syndrome, Preterms, Hydrocephalus, Rickets, Congenital rubella, Congenital syphilis, hondroplasia, PEM, Cutis laxa, Hypophosphatasia, Mucopolysaccharidoses.

2. Regarding Rabipur, it is a safe vaccine and would not cause any neurological problems as with the older rabies vaccine.

3. For mesenteric lymphadenitis, one has to find the cause and treat accordingly. Rule out TB, HIV, yersinia and even other viral infections.
