How should I approach an epidemic of pertussis in an area?
Answer
Complete immunization of all children is the most important preventive measure in achieving and maintaining control of pertussis. Children exposed to infected people should have their immunization status reviewed. If immunization is incomplete and unless there are contraindications the necessary vaccine doses should be given as follows: (a) children who have received fewer than three doses should receive their additional dose(s) as soon as possible every 4 weeks, (b) children who have had three doses may receive the fourth one as early as 6 months after the third dose, and (c) children 6 years of age or under who have had four doses of pertussis vaccine should be given a booster dose, usually as diphtheria toxoid-pertussis vaccine-tetanus toxoid (DPT), unless the most recent dose was within the past 3 years.
Passive immunization with immune globulin has no place in the management of pertussis.

Erythromycin given to contacts within 2 weeks after the onset of symptoms in the index case halts transmission and prevents disease in contacts.
