Question
In the recent past I have seen few patients of diphtheria coming for medical attention within days of developing symptoms. Can I use injection erythromycin in them for treatment. Within how many days of developing symptoms can I give/ use erythromycin in them?
Answer
Specific antitoxin is the mainstay of therapy. Antimicrobial therapy is indicated to halt toxin production, treat localized infection, and prevent transmission of the organism to patient contacts. Penicillin and erythromycin only are recommended for treatment. Erythromycin is marginally superior to penicillin for eradication of nasopharyngeal infection.
Erythromycin dose: 40-50 mg/kg/d PO/IV divided tid/qid; not to exceed 2 g/d; treatment is repeated if culture results are positive.
