Question
A breast feeding mother having a two month infant develops Chicken pox. How is the infant and mother to be managed? Is Acyclovir to be given to the mother? Is the infant to be given VZIG/Acyclovir?
Answer
Treatment is only given to those with severe disease or at risk of severe disease. Thus acyclovir in mother would depend on the clinical symptoms. In the child, if exposed and infected upto one month of age, VZIG and acyclovir are recommended. However, in infants more than one of age only and with history of exposure to chickenpox, VZIG should be given within 96 hours of exposure to prevent infection as the child would not have transplacental acquired antibodies since the mother is not immune to the disease. Acyclovir would be required only is the child forms a severe disease.
