Except STM all other anti TB drugs are available for per oral administration. How to administer ATT in patients with following problems? 1. A patient diagnosed as a case of abdominal kochs but has uncontrollable vomiting inspite of being given injectable antiemetics. 2.A case of TBM, having vomiting and altered sensorium. Answer
It there is persistent vomiting, it is necessary to find out the cause. Rule out intestinal obstruction and hepatitis. Also ni TBM rule out raised ICT. As the child may require at least 3-4 drugs during intensification phase, it is necessary to give all drugs together. Thus one may have to wait till the vomiting stops to start the ATT. In case vomiting persists, then streptomycin, ciprofloxacin may be tried. Other option is to put a gastrojejunostomy tube.
