Question
I have got this patient who came in with rash, bilateral large cervical lymphadenopathy, fever, tonsilitis, andlethargy. Tonsils had exudates and he was treated as Infectious mononucleisis. In two days he developed glomerulonephritis with edema, hematuria,mild protenuria,hypertension and was started on antihypertensive nifedipine and also lasix tds. But today his platelets are going down to 85 and he started to have petechiae with no bleeding symptoms. So what could be the diagnosis?
Answer
Rule out post streptococcal AGN. Get a culture swab of the tonsils. Also do ASLO. If thrombocytopenia persists, watch out for HUS.
