Usually the breast milk output decreases as the baby is weaned and usually by 1 year of age when the baby is on full solids, the milk requirement comes down. When the baby suckles less on the breast, the milk output will automatically decrease.
For pain, you may try hot water formentation and if it persists then painkillers such as paracetamol may be taken.
