Question
My 3 year old daughter is suffering with a very painful stiff neck and fever. Two nights ago she went to bed with a headache then woke in the night with fever and headache. I gave panadol and she resettled till morning when she awoke still with the headache, fever and stiff, painful neck. We were advised to take her to a&e where they ruled out meningitis but could not explain her symptoms. We came home an though the headache is gone the very painful neck remains. I am very concerned and other than gibe her child nurofen am unsure how to help her. What do you advise? She will not let me touch her neck even lightly as it makes her wince and she is unable to look up or rotate her neck to the side. any suggestions? Thank you for your help.
Answer
She could be having an inflammation in the spine such as discitis or spondilitis or even tuberculosis. An X ray of the spine would be useful. At the moment she would require a cervical collar to prevent excessive movement of the spine and analgesics to decrease the inflammation. Depending on the investigations she may require further tests such as MRI of the spine. You may also need to get her throat examined to rule out a retropharyngeal abscess or even tonsillitis as they also cause a painful neck and fever.
