Question
Our grandson (now 14 months old) was been diagnosed with Respiratory Airway Disease at the age of 2 months. The doctors (pediatric pulmonologist and gastro-interologist) are now wanting to perform surgery on his stomach to prevent reflux which they feel was causing the RAD. My concern is that while he has had some minor problems with colds this winter - there has been no instance of pneumonia or a serious case of RAD. They have also mentioned that an allergy to milk could be part of the problem with the reflux. Is there any way to treat the reflux without resorting to a surgical procedure to turn his stomach?
Answer
The treatment for GER or reflux consists of diet changes, drugs and finally surgery in that order. In most patients thickening of the feeds, putting the baby in the prone position with the head held high after feeds is only what is required. Most of the babies overcome their GER by 2 years of age. If the child still persists to have reflux, reactive airway disease and failure to thrive, motility drugs such as cisapride are used. If the child has severe GER (grade 4 or more as per the milk scan), frequent pneumonias, failure to put on weight and failure of medical management then only surgery is opted for. What you need to do is discuss your grandson's problem with his doctors and understand why he needs to go for surgery.
