Question
I have a 7 year old daughter that has suffered from recurrent UTI's since the age of3. The etiology is yet unknown and has undergone multiple of the above mentioned tests. I would appreciate any further input you have on her condition and possible options in treatment or discovery of the problem. They are all e-coli with systemic reactions, including but not limited to fever, headache, cold sores, foul smelling breath, abdominal cramping. She is now on prophylaxis with ceftin and can no longer use bacterium because she has developed a severe allergy. We have been to multiple physicians including a urologist, pediatrician and nephrologist. Would it be a wise choice to include an ID gentleman at this time?
Answer
The cause of UTI in children are several. The commonest in children below 5 years of age are due to birth defects causing renal malformations. These can be detected by the various tests such as ultrasound of the kidneys, nuclear scan and GU workup (MCU test). Sometimes the cause is due to VUR (vesico urethral reflux) where the urine comes back into the ureter when the bladder contracts. This can be detected by MCU. Sometimes the cause of UTI may be not known and the child may suffer from UTI. Such children with recurrent UTI require long term prophylaxis with antibiotics to prevent recurrence. You are rightly consulting a pediatrician, nephrologist and urologist for you child. That is the ideal team to treat pediatric UTIs.
