Question
My son is suspected to have juvenile rheumatoid arthritis. He is 12 y/o. His blood test result show that he has a high ESR value (initially 135 then down to 113) i wish to request you to explain to me the meaning of this and what is the indication of this result. he also has high WBC and platelet count. He was given NSAID, methotrexate and folic acid for his therapy. Thank you
Answer
A high ESR is just a marker of acute inflammation. It tells you about the disease activity (a high value suggests that the disease is still active and not yet in remission). ESR helps in monitoring the treatment and also helps to determine whether the child is responding to the treatment or not. A high WBC count and a high platelet count are all seen when the JRA flares up. Your child's ESR is coming down but it may be weeks before it becomes absolutely normal.
