Question
I am wondering what rheumatic fever is? What are the symptoms? What is the treatment?
Answer
Rheumatic fever is an acute inflammatory disease which follows streptococcal infection of the throat. Its most common manifestation is arthritis, most serious- carditis (heart involvement), most curious- nervous system involvement leading to involuntary movements and most rare and inconsequential- skin rash. It is seen in children between 5-15 years of age. The treatment for arthritis and carditis is bed rest and anti-inflammatory drugs. Heart involvement in addition would require drugs to control the heart failure, aspirin and maybe even steroids depending upon the severity. To prevent the recurrence of the attack, penicillin intramuscular or orally is recommended for a long time, maybe even for the lifetime. The outcome has a wide range of possibilities -from total recovery (with risk of recurrences) to death due to cardiac failure. In between there are various kinds of chronic rheumatic heart diseases. Patients with no carditis have the best prognosis.
