My brother's grand daughter, of age 2 years, has hearing and speech problem. Infact she cannot speak even simple words like Aai,Baba, Kaka,etc. Her responses to our conversation are also very slow. We have fixed hearing aids in both her ears for last 4-5 months but without much usefulness. I would like to know answers for the following : 1] What are the clinical / physiological test necessary to be done to have proper diagnosis and where these tests could be done? 2] Would the surgery overcome the defect and what are the chances of her proper listening and speaking? 3] Is the defect a hereditary one? 4] Who could carry out the surgery and where? Thanks.
Answer
First and foremost one needs to evaluate the cause of her delayed speech. Is it just hearing loss or does she have other milestones that are also delayed? Did she have any problems at the time of birth? If she has had any problems at birth, then the speech would depend not only on the hearing loss, but also on the brain damage that may have occurred. If her other milestones are also delayed then she needs evaluation for her delayed milestones. She would need to be evaluated by a pediatrician thoroughly. IF it is just hearing loss, then her hearing would need to be tested when she has the hearing aids and check if it is normal. Also, she may need some sort of speech therapy like play therapy to improvise her speech. For that you would need to consult a speech psychologist. Thus, in short get her evaluated by a pediatrician and a speech therapist and the management required would be a combined effort. Regarding surgery, it would depend on the cause and then treatment for the same such as sensorinerual deafness may improve with cochlear implants. However, first a clear diagnosis is required.
