Question
I help take care of my grandson who is now 6yrs. old. He was a preemie, born at 29 weeks gestation. He has been developmentally delayed and diagnosed with mild cerebral palsy. He now walks, runs, and is learning to speak in sentences. He didn't start to speak until about 4yrs. old. He is very intelligent and is quite proficient in computer operations. He spends hours at the computer. His teachers praise his progress. however, the big problem is that he is not potty trained. He tries to sit on the toilet and move his bowels but he has never succeeded.also, the same thing with urination. He seems to want to be able to urinate in the toilet but stands there and nothing happens. It is becoming a real problem now because the teachers think he can progress to the first grade. He will be seven in July. The teachers say he must be toilet trained. We have never had the doctors comment on this. We are beginning to think could this be a problem because of the cerebral palsy. Could you please suggest what we should do.Thanking you in advance.
Answer
Does he get the bladder and bowel sensation (that he wants to pass urine or stool)? Also can he hold back the urine when he gets the urge? What is the amount he passes at each time? Is he dry during the night? If all that is normal, he just needs to be trained more aggressively. If not, he would need evaluation in the from of bladder/sphincter dyssynergia and maybe even a dynamic study to check for the bladder and bowel dysfunction. ALso an X ray of the spine to look for occult spina bifida would be useful.
