A two year old boy was diagnosed with ITP. He was given Prednisolone 10mg for two months and then 5mg for the third month and tapered off in the 4th month gradually. After discontinuation he developed fever with purpura again. What to do? How to discontinue Prednisolone sans withdrawal effects? Are there natural steroids in Ayurveda which can replace Prednisolone? How to treat the withdrawal symptoms with Ayurveda or Homeopathy? Kindly let me know. Will taking of Prednisolone for four months effect growth of this child? Also I would like to know whether the growth retardation is permanent in small children of this age as a result of taking Prednisolone.
Answer
You need to get the child evaluated for chronic ITP as he seems to have the illness going on for more than 4 months. Also, if it is chronic ITP, then other medications such as IVIG or Anti D may be required. Long term prednisolone given in high doses over a period of many months may lead to growth retardation, however, it has to be given under a doctor's guidance monitoring the side effects. If the growth velocity is OK and there is no evidence of bone weakness, cataracts or muscle weakness, it can be given safely.
