Question
My 8 month old nephew has an ear infection. Methicillin resistant staph, among other bacteria, was cultured. He was treated with IV Vancomycin, MRS was cultured again, so tubes were removed from ears & he was restarted on the Vanco. He had a severe allergic reaction on the second treatment. Now other antibiotics are being debated. Any suggestions?
Answer
Methicillin resistant staphylococcal infection is a very difficult infection to eradicate. Though Vancomycin is the drug of choice, you may need to do the antibiotic sensitivity pattern of the organism grown along with the MIC (minimum inhibitory concentration). Depending upon the sensitivity and MIC, other antibiotics such as Teicoplanin, Rifampicin or other antistaph antibiotics can be tried. What was the allergic reaction to vancomycin? If the allergy was not severe, vancomycin could still be the drug of choice
