Question :
What are the advantages & disadvantages of radiotherapy in retinoblastoma for one eye & its size recently 13.5/3.6 mm for my son Atul (date of birth 16/4/2001). While right eye is healthy. Treatment is going on AIIMS, Delhi since 28/2/021. 12 cycle of chemo has completed on 14/7/03 & two injections of carbo in defect eye. Others tests related or unrelated are ok. They will re-exam the eye after 6 months. I have now two options one is eye removal & another is radiotherapy. Can you give me your kind suggestion which one is best & is threatment going in correct direction? Thank you.
Answer
Since most unilateral disease is usually massive and there is often no expectation that useful vision can be preserved, surgery (enucleation) is usually undertaken and radiation therapy is not given to the tumor bed.

When there is potential for preservation of sight because the tumors are smaller, treatment with other modalities (radiation therapy, photocoagulation, cryotherapy, thermotherapy, chemoreduction, and brachytherapy) instead of surgery should be considered. However, treatment planning by a multidisciplinary team of cancer specialists who have experience treating ocular tumors of childhood is required to determine and implement optimum treatment. Because of the complexity of therapy, expertise in pediatric radiation therapy and ophthalmology should be available.

Your oncologist and ophthalmologist would be the best guide for your son.
