Question
I have a 5 yr. old boy who's been sick for 14 days now. Symptoms are fever (around 100-101), leg and arm pain, stomach pain, headache, vomiting on occasion, and diarrhea. He's been to the doctor twice; the second time was in the emergency room for an i.v. and blood work. They haven't found a bacterial infection, they think it's viral. I have to give him Tylenol Motrin every 4 hours or his fever and pain comes back. He has problems with walking because of the pains in his legs. He lies around all day with no energy. Is this par for the course with viral infections? He's never been this sick before and I'm very concerned. They have ruled out meningitis.
Answer
Poikilothermia may be seen with holoprosencephaly due to affection of the hypothalamus. If the fever does not come down with antipyretics, a central cause of fever such as a brain lesion is suspected provided infection, inflammation and malignancy has been ruled out. Treatment consists of NSAIDS such as paracetamol and ibuprofen.
