Beclomethasone
Mechanism :
Beclomethasone dipropionate is a diester of beclomethasone, a synthetic corticosteroid chemically related to dexamethasone. Corticosteroids have multiple anti-inflammatory effects, inhibiting both inflammatory cells and release of inflammatory mediators. It is presumed that these anti-inflammatory actions play an important role in the efficacy of beclomethasone dipropionate in controlling symptoms and improving lung function in asthma.
Indication :
- Prophylaxis of asthma
- Prophylaxis and treatment of allergic rhinitis
Contraindications :
Beclomethasone can occasionally cause paradoxical bronchospasm. Use with caution in respiratory infection.
Dosing :
Nasal Inhalation for allergic rhinitis: 1 puff = 50 mcg.
1-2 inhalations in each nostril twice daily.
Oral inhalation for asthma:
5 to 12 years: 50 mcg inhaled twice daily. Maximum dose: 100 mcg twice daily.
>12 years: 50-100 mcg inhaled twice daily. Maximum dose: 400 mcg twice daily.
Adverse Effect :
Urticaria, angioedema, rash, and bronchospasm, systemic adrenal suppression, oral candidiasis (with inhalers).
Interaction :
Interactions like other corticosteroids amongst which the most significant recently is:
Inhaled corticosteroids are substrates of hepatic CYP3A4 and have the potential to interact with ritonavir. These interactions can result in steroid accumulation, adrenal suppression and Cushing's syndrome.
Hepatic Dose :
No dosage adjustments are recommended.
