Formoterol Fumarate
Mechanism :
The pharmacologic effects of beta2-adrenoceptor agonist drugs, including formoterol, are at least in part attributable to stimulation of intracellular adenyl cyclase, the enzyme that catalyzes the conversion of adenosine triphosphate (ATP) to cyclic-3', 5'-adenosine monophosphate (cyclic AMP). Increased cyclic AMP levels cause relaxation of bronchial smooth muscle and inhibits the release of pro-inflammatory mast-cell mediators such as histamine and leukotrienes.
Indication :
- Long-term maintenance treatment of asthma in patients 6 years of age and older
- Prevention of exercise-induced bronchospasm,
Contraindications :
Formoterol fumarate is contraindicated in patients with a history of hypersensitivity to formoterol fumarate or to any components of this product.
Dosing :
6 years or older:
12 mcg (1 inhalation) of powder at least 15 minutes before exercise as needed or 12 mcg every 12 hours in case of long term maintenance of asthma along with inhaled steroids.
Adverse Effect :
Hypertension or hypotension, angina, tachycardia, palpitations and arrhythmias, hyperglycemia, rash, muscle cramps, dry mouth, nausea, headache, nervousness, dizziness, tremor, insomnia, hypokalemia, metabolic acidosis, bronchitis, chest infection, dyspnea, tonsillitis and dysphonia, hypersensitivity reactions.
Interaction :
Xanthine Derivatives, Steroids or Diuretics: May potentiate hypokalemia.
Disopyramide and Tricyclic Antidepressants: may be associated with QT interval prolongation and increased risk of ventricular arrhythmia.
Beta-blockers: May weaken or antagonize the effect of formoterol.
